The strongest evidence present is Tier 1: a large observational cohort of 266 elderly persons over 6-8 years (variously described as observational and, by one vendor, as a single adequately-powered RCT, PMID 14523363) plus tier-1 reviews and psoriasis/endometrial clinical studies. However, most human data are observational rather than blinded RCTs, nearly all research originates from a single Russian group (Khavinson laboratory) with limited independent Western replication, and pharmacokinetic and specific mortality-percentage figures come only from tier-3 vendor material. Thymalin is an undefined, batch-variable bovine thymus extract with a theoretical prion/contamination risk and no FDA/EMA approval.

How it works

Sources describe Thymalin as a polypeptide complex isolated from the thymus gland of young calves that acts as an immune regulator. Khavinson-group sources report it contains short peptides (the dipeptides EW and KE and the tripeptide EDP) that are said to bind DNA and histone proteins to influence which genes are switched on, thereby helping bone-marrow-derived precursor cells mature into T-lymphocytes and restoring balance to the immune system. Sources report that Thymalin (also called Thymus peptide complex or Thymalinum) is a polypeptide complex isolated from the thymus gland of young calves (bovine calf thymus) (src-3, src-6, src-7, src-8). Vendor sources report it was developed in the 1970s in the Soviet Union by Vladimir Khavinson and Vyacheslav Morozov at the Military Medical Academy in Leningrad, with a tier-2 study noting Khavinson characterized these peptides from 1973 onwards (src-7, src-9, src-12, src-22). Sources give varying regulatory dates: one states first USSR approval in 1982 for immunodeficiency states, others say Russia since the 1980s or 1970s (src-7, src-9, src-13, src-4); Khavinson-group studies and a vendor cite a Russian registration number LS-000267 dated February 26, 2010 (src-3, src-6, src-7).

A tier-1 review describes Thymalin as a polypeptide drug used for diseases associated with immune dysfunction, viral and bacterial infections, normalization of regeneration, immunodepression, and depression of hematopoiesis following chemical and radiotherapy (src-1).

Composition and classification

Multiple Khavinson-group sources report Thymalin's components include the dipeptide EW (thymogen), the dipeptide KE, and the tripeptide EDP (src-1, src-3, src-6, src-7, src-14). A vendor reports the EW dipeptide has molecular formula C16H19N3O5 and molecular weight 333.34 g/mol (src-7). Multiple vendor sources describe Thymalin's molecular weight as roughly 1,000-10,000 Da\of of short peptides of 2-8 amino acids (src-7, src-8, src-12, src-9). Khavinson-group sources state manufacturing uses special extraction and purification that completely degrades macromolecular proteins while preserving peptides under 10 kDa (src-2); a vendor reports it is produced by acid hydrolysis of bovine calf thymus tissue followed by ultrafiltration to isolate peptides below 10 kDa (src-7). A vendor classes Thymalin among the cytomedin family of short peptide bioregulators that influence DNA transcription and cellular differentiation (src-13).

Reported mechanism

Khavinson-group studies report that the short peptides KE, EW and EDP can specifically bind double-stranded DNA and/or histone proteins to regulate gene expression, immune-protein synthesis, gerontogene activity and stem-cell differentiation (src-3, src-6). A tier-2 docking study reports GGAG as the preferred DNA sequence for EW and GCGC for KE, and identified AKT1/AKT2 as potential target genes involved in cytokine-storm development (src-20). In vitro, Thymalin and its EW and KE dipeptides reduced IL-1beta, IL-6 and TNF-alpha synthesis in human PBMCs by 1.4-6.0 times (src-20), and a THP-1 monocyte study reported inhibition of LPS-stimulated TNF and IL-6 and induction of TNF tolerance (src-22). Khavinson-group and tier-2 reports describe modulation of hematopoietic stem cell markers (CD44, CD117 down; CD28 up) interpreted as stimulating differentiation into mature T-lymphocytes (src-3, src-6, src-18, src-4). A tier-1 review states the immunoprotective effect is expressed as restoration of T and B lymphocytes and their subpopulations, increased functional activity and increased phagocytosis (src-1). One vendor theorizes its polypeptide composition produces broader but less targeted effects than single-sequence peptides (src-10, src-18).

Reported clinical and preclinical evidence

A tier-1 study reports that in 266 elderly and older persons observed over 6-8 years, Thymalin and Epithalamin improved indices of the cardiovascular, endocrine, immune and nervous systems, homeostasis and metabolism (src-2, src-19, src-30). The same dataset reports Thymalin alone produced a 2.0-2.1-fold decrease in mortality versus control, Epithalamin alone a 1.6-1.8-fold decrease, the combination a 2.5-fold decrease, and the combination applied annually for 6 years a 4.1-fold decrease (src-2, src-19, src-30). It is worth noting these primary sources describe the study as observational, while one vendor (src-4) characterizes it as a single adequately-powered RCT (PMID 14523363). Separately, a tier-3 vendor (src-9) characterizes the trial as producing a 45% reduction in cardiovascular mortality and a 28% reduction in all-cause mortality ,'s figures that appear only on that single site. Tier-1 studies also report Thymalin was associated with a 2.0-2.4-fold decrease in acute respiratory disease incidence and reduced incidence of ischemic heart disease, hypertension, deforming osteoarthrosis and osteoporosis (src-1, src-2, src-19, src-30).

Khavinson-group studies report that in COVID-19, Thymalin plus standard therapy (n=36) produced more rapid clinical improvement than standard therapy alone (n=44), with faster recovery from lymphopenia and normalization of C-reactive protein, D-dimer, lymphocyte and NK-cell counts, and that it halved hospital mortality in older patients with severe COVID-19 (src-3, src-6, src-14). A case report describes a 2-fold increase in lymphocytes, 7-fold in eosinophils, and reductions of 8x in CRP, 2.5x in IL-6 and 7.2x in D-dimer (src-3, src-6). A tier-2 study describes efficacy in acute respiratory distress syndrome and COPD and in complex therapy of severe COVID-19 (src-20, src-18). Khavinson-group sources report use in hepatitis A and B, erysipelas, meningococcal infection, typhoid fever, pneumonia and cavernous pulmonary tuberculosis (src-3, src-6).

Additional tier-1 clinical reports include a study of 104 psoriasis patients showing decreased T-helper/T-suppressor ratios versus control, and 27 psoriatics treated with Thymalinum plus Natrii nucleinas whose ratio rose from 1.0 to 1.74 with shorter rash regression and prolonged remissions (src-24), and use of Thymalinum with norcolut and myelopidum in 55 patients with hyperplastic endometrial processes (src-25). Animal studies (mice, rats, Drosophila, 1971-2001) are reported to prolong lifespan and inhibit age-related disorders (src-2), with a vendor citing 20-30% lifespan extension in animal models (src-13). Tier-2 animal studies report a pronounced antitumor effect against transplanted sarcoma 45 in rats (src-21) and activation of reparative osteogenesis around a jaw bone defect in WAG rats (src-26). Tier-3/4 sources describe potential roles in correcting radiative immunodeficiencies (src-17), dental and maxillofacial pathology (src-29), and athlete recovery and anti-aging protocols (src-15).

Evidence caveats

A vendor profile characterizes the overall evidence base as roughly 73% preclinical and 27% clinical, with an Evidence Score of 57/100, noting most studies come from a single Russian group (the Khavinson laboratory) with limited Western replication (src-4); another vendor counts 234 published studies and notes Thymalin is approved in Russia but never evaluated by Western regulators and is an undefined bovine thymus extract with batch-consistency challenges (src-8). Readers should note that nearly all evidence originates from one research group, most human data are observational rather than blinded RCTs, and sources disagree on whether Thymalin is a natural extract or synthetic and on its approval dates.
